Our Story.
ThreadConnect didn’t begin in a Silicon Valley lab or from a generic software idea. It grew out of the experience of leaders working in aging and community-based services who saw firsthand how fragmented systems were creating unnecessary work for staff and making coordination harder for the people they serve.
They saw organizations doing increasingly complex work across programs, partners, healthcare systems, and government agencies - often without technology designed around the realities of community-based care.
Rather than asking organizations to adapt to technology built for someone else, they set out to build something around the way CBOs actually work.
That idea became ThreadConnect.

Backed by Leaders in Aging & Community Care
ThreadConnect was created through a partnership between Nevada Senior Services (NSS) and the American Society on Aging (ASA), with contributions from leaders and organizations across aging, healthcare, technology, and community-based services.
Together, those perspectives brought operational experience, sector knowledge, and a deep understanding of the realities facing community-based organizations.
ThreadConnect was not designed around an abstract vision of how care coordination should work.
It was shaped by how the work actually happens.
